MEDICAL AND HEALTH INSURANCE

 Eating - all tasks of getting food into the body once it has been

                       prepared.

When the above benefits are triggered, the insurer may pay a daily or

monthly fixed allowance or a lump sum payment as specified in the plan.

               DENTAL INSURANCE

Dental expense insurance is generally sold as part of the group PMI

extended benefits. However, many insurers do not offer this type of cover
due to lack of demand and the costly premium and there is no individual

               standalone policy.

The benefits offered are generally for preventive care (cleaning, scaling, x-

rays, etc.) and non-routine care, as in, repair and restorative (filling, root

canal, etc.). The limits are generally pre-fixed and specified in the dental
               plan schedule.

Preventive care will generally provide coverage for:

               a) consultation, for dental examination and diagnosis

               b) teeth cleaning or scaling every 6 months

               c) diagnostic X-rays, etc.

Non-routine care will generally provide coverage for:

               a) restorative or repair of damage, for example filling or extraction of a

               tooth or root canal treatment

               b) oral surgery in the oral cavity, for example, as in the removal of wisdom

               tooth.

For non-routine care, there may be a deductible or co-insurance imposed,

especially if there is no pre-fixed amount set and subject to customary and
